Kindergarten Policies & Forms

Picking up your child

If you require some one else to pick up your child, please ensure you have them listed as either an Additional Permanent Authorisation to Collect or an Additional Temporary Authorisation to Collect.  You can download the forms here and give to your child's teacher or administration staff.

Infectious Disease

Absence through Illness

Kindy is not the place your child wants to be when they are unwell.

Please contact Vera Lacaze and inform us of any infectious illness your child may have contracted so that we can inform other caregivers.  Your information will be kept confidential.

It is always better to check with a health care professional on when your child can resume Kindy.

It is recommended that you refrain from visiting the centre with your child or their siblings when they are not in good health.

Allergy Notificiation

Immunisation Policy

Sundry Forms

It is the parent's responsibility to notify the centre of any allergies their child is at risk of.

Parents must provide the centre with a written and current independent medical report that outlines the child's medical condition, the recommended treatment able to be provided by Vera Lacaze staff and any other matters deemed relevant (including an Anaphylaxis Action Plan).

Prior to enrolment and attendance at Vera Lacaze Memorial Community Kindergarten, parents are required to provide an immunisation history statement stating that their child's immunisation is 'up to date' (and an updated statement when their child passes future vaccination milestones if applicable).

Children who are on an approved immunisation catch-up schedule developed by a recognised immunisation provider are also considered 'up to date'.

Parents who choose to not vaccinate their children are required to complete a Conscientious Objection to Immunisation form and return it to our Administration staff.
